Dear Valued Patient, 

Welcome to Kare KONEC Medial and Urgent Care Centre 24-7; we are happy that you have chosen  us as your healthcare provider. Our mission is to deliver high-quality care to all our patients in a  timely and courteous manner. 

We will do our best to accommodate same-day office visits and welcome walk-ins for urgent care  visits. For each appointment, you will need to bring your insurance card and a valid ID. Please let  us know if your information has changed since your last appointment. If you are unable to present  us with your insurance card, you will be invoiced self-pay. Also, you will be prompted to fill out  new registration forms on an annual basis so that we can update your information.  

Please bring all your prescription and over-the-counter drugs with you to each visit so our  physician can effectively examine your health and determine efficient care coordination with  other healthcare providers for treatments regarding your health. 

We prioritize providing our patients with the highest quality professional care possible. As a result,  it is important to know that our care management staff will contact you within 48 hours after discharge to follow up and examine your health and recovery status. Always remember to phone  119 for actual life threating emergency situations or visit our urgent care clinic 24-7.
